ABSTRACT

Abstract Objective Translate and cross-culturally adapt into Brazilian Portuguese the Glasgow Children's Benefit Inventory instrument used for the quality-of-life assessment after pediatric ENT interventions. Method This is a methodological study of translation and cross-cultural adaptation of the GCBI instrument following seven stages: 1) Translation of two versions by two independent translators, 2) Elaboration of a consensual synthetized version, 3) Assessment of the synthetized version by experts, 4) Assessment by the target audience, 5) Back-translation, 6) Pilot study and 7) Use of the instrument. The final version of the instrument was answered by a sample of 28 people responsible for children aged from 2 to 7 years, submitted to tonsillectomy between January 2019 and December 2021, in a public hospital in Porto Alegre. The collection considered patients with a minimum of 6-months and a maximum of 3-years of postoperative follow-up. Result The instrument final version was compared to the original version showing semantic equivalence, absence of consistent translation difficulties and appropriate cross-cultural adaptation, and well understood by the target audience. The application of the questionnaire in the sample showed a Cronbach alpha coefficient of 0.944 corresponding to a high degree of reliability of the instrument. Conclusion The translation and cross-cultural adaptation showed semantic appropriateness and its use when assessing ENT postoperative results in a pediatric population showed high reliability of the instrument. ABSTRACT Objective: To evaluate the occurrence of adverse events in the postoperative period of cardiac surgery in a pediatric intensive care unit and to find any patient characteristics that can predict such events. Methods: This was a historical cohort study of patients recovering in the pediatric intensive care unit for the first 7 days after cardiac surgery between April and December 2019, by reviewing the medical records. The following were reviewed: demographic, clinical, and laboratory characteristics; patient severity scores; and selected adverse events, grouped into device-related, surgical, and nonsurgical. Results: A total of 238 medical records were included. At least one adverse event occurred in 110 postoperative patients (46.2%). The total number of adverse events was 193 (81%). Vascular catheters were the most common cause, followed by cardiac arrest, bleeding, and surgical reexploration. In the univariate analysis, the vasoactive-inotropic score (VIS), Risk Adjustment in Congenital Heart Surgery (RACHS-1) score, age, Pediatric Index of Mortality (PIM-2), cardiopulmonary bypass and aortic clamping duration were significantly associated with adverse events. In the multivariate analysis, VIS ≥ 20 (OR 2.90; p = 0.004) and RACHS-1 ≥ 3 (OR 2.11; p = 0.019) were significant predictors, while age and delayed sternal closure showed only trends toward significance. To predict the occurrence of adverse events from VIS and RACHS-1, the area under the curve was 0.73 (95%CI 0.66 - 0.79). Conclusion: Adverse events were quite frequent in children after cardiac surgery, especially those related to devices. The VIS and RACHS-1, used together, predicted the occurrence of adverse events well in this pediatric sample.


RESUMO Objetivo: Avaliar a ocorrência de eventos adversos em pós-operatório cardíaco em uma unidade de terapia intensiva pediátrica e estabelecer eventuais associações das características dos pacientes e a possibilidade de predizer tais eventos. Métodos: Coorte histórica de 7 dias de pós-operatório cardíaco, de abril a dezembro de 2019, por revisão de prontuários de pacientes com recuperação em unidade de terapia intensiva pediátrica. Foram revisados: características demográficas e clínico-laboratoriais, escores de gravidade dos pacientes e eventos adversos selecionados agrupados em: relacionados a dispositivos, a aspectos cirúrgicos e a aspectos não cirúrgicos. Resultados: Foram incluídos 238 prontuários. Ocorreu pelo menos um evento adverso em 110 pós-operatórios (46,2 %). O número total de eventos adversos foi 193 (81%), sendo mais frequente a complicação com cateteres vasculares, seguida de parada cardíaca, sangramento e reexploração cirúrgica. Na análise univariada, escore vasoativo-inotrópico (VIS- vasoactive-inotropic score), Risk Adjustment in Congenital Heart Surgery (RACHS-1) score, idade, Pediatric Index of Mortality (PIM-2), tempo de circulação extracorpórea e de clampeamento aórtico foram estatisticamente significantes com eventos adversos. Na análise multivariável, VIS ≥ 20 (OR 2,90; p = 0,004) e RACHS-1 ≥ 3 (OR 2,11; p = 0,019) mostraram-se relevantes e com significância estatística, enquanto idade e fechamento tardio do esterno possuíam apenas tendência a essa associação. Considerando a previsão de ocorrência de eventos adversos a partir dos valores de escore vasoativo-inotrópico e de RACHS-1, a área sob a curva mostrou valor de 0,73 (IC95% 0,66 - 0,79). Conclusão: A frequência de eventos adversos foi expressiva e aqueles relacionados a dispositivos foram os mais frequentes. O VIS e o RACHS-1, utilizados em conjunto, foram capazes de predizer a ocorrência de eventos adversos nesta amostra pediátrica.

Introducción: La intervención quirúrgica cardiaca es compleja, pero con resultados satisfactorios para el paciente, pues incrementa su supervivencia y calidad de vida. El síndrome de bajo gasto cardiaco en el periodo posoperatorio es una de las entidades que ensombrece el pronóstico y eleva los índices de morbilidad y mortalidad en dichos pacientes. Objetivo: Estimar la supervivencia de los pacientes con síndrome de bajo gasto cardiaco en el periodo posoperatorio según variables clínicas y ecocardiográficas. Métodos: Se realizó una investigación de cohorte de supervivencia en 56 pacientes operados del corazón, diagnosticados con síndrome de bajo gasto cardiaco posoperatorio, quienes fueron atendidos en el Centro de Cardiología y Cirugía Cardiovascular de Santiago de Cuba, desde enero del 2019 hasta noviembre del 2021. Resultados: Predominaron el grupo etario de 65 o más años (60,7 %), la diabetes mellitus (44,6 %) como antecedente patológico personal y los pacientes con fracción de eyección de 45 % o más, quienes presentaron mayor cantidad de decesos (29,8 %). La función del ventrículo derecho afectada se halló en 52,6 % de los fallecidos. El tiempo de circulación extracorpórea de 90 minutos o más primó en 67,9 % de los afectados, de los cuales 42,1 % murieron. Conclusiones: Se observó que la diabetes mellitus, el tiempo de circulación extracorpórea y el sangrado perioperatorio prolongados, así como la función sistólica biventricular se relacionaron con el pronóstico del síndrome de bajo gasto cardiaco en el periodo posoperatorio.


Introduction: Heart surgical intervention is complex, but with satisfactory results for the patient, because it increases his survival and life quality. The low cardiac output syndrome in the postoperative period is one of the entities that darkens the prognosis of this operation and elevates the morbidity and mortality indexes in these patients. Objective: To estimate the survival of patients with low cardiac output syndrome in the postoperative period according to clinical and echocardiographyc variables. Methods: A cohort investigation of survival in 56 operated patients was carried out, who were diagnosed with postoperative low cardiac output syndrome and were assisted in the Cardiology and Cardiovascular Surgery Center of Santiago de Cuba, from January, 2019 to November, 2021. Results: There was prevalence of the 65 years and over age group (60.7%), the diabetes mellitus (44.6%) as personal pathological history and patients with ejection fraction of 45% or more who presented higher quantity of deaths (29.8%). The function of the affected right ventricle was found in 52.6% of deaths. The time of extracorporeal circulation of 90 minutes or more prevailed in 67.9% of those affected, of whom 42.1% died. Conclusions: It was observed that diabetes mellitus, the time of extracorporeal circulation and the long perioperative bleeding, as well as the biventricular systolic function was related to the prognosis of low cardiac output syndrome in the postoperative period.

El ejercicio sigue siendo una intervención necesaria para la pérdida y mantenimiento del peso en pacientes postquirúrgicos de cirugía bariátrica. Sin embargo, los principios básicos de su prescripción luego de este tipo de intervenciones son aún controversiales en la literatura. En este contexto, el objetivo de este estudio fue describir los principales componentes de la prescripción del ejercicio en pacientes postquirúrgicos de cirugía bariátrica y sus beneficios asociados. Se realizó una revisión exploratoria usando las bases de datos Pubmed, PEDro, Springer, Science Direct, y Scielo. Se utilizó un algoritmo de búsqueda con los términos clave "cirugía bariátrica", "obesidad", "ejercicio", "post-quirúrgico", "prescripción del ejercicio" y "actividad física", en inglés y español. Fueron incluidos ensayos clínicos y estudios observacionales publicados entre 2010-2020, donde hubo prescripción del ejercicio posterior a cirugía bariátrica. Un total de 13 estudios fueron analizados. Los parámetros recomendados incluyeron la frecuencia de 3 a 5 veces por semana, intensidad aeróbica entre el 60-85% de la FCmáx o 60-70% de VO2máx, intensidad anaeróbica entre el 60-85% de 1 repetición máxima (1RM), y duración entre 12 y 18 semanas. El ejercicio es una herramienta importante para mantener y reducir el peso, atenuar el sedentarismo y mejorar la calidad de vida en paciente postquirúrgicos de cirugía bariátrica con resultados seguros y eficaces siempre y cuando se garanticen los niveles de intensidad, frecuencia, duración, tipo y progresión sugeridos.


Exercise remains a necessary intervention for weight loss and weight maintenance in post-bariatric surgery patients. However, the basic principles of its prescription after this type of interventions are still controversial in the literature. In this context, our objective was to describe the main components of exercise prescription in post-bariatric surgery patients and its associated benefits. A scoping review was carried out using the Pubmed, PEDro, Springer, Science Direct, and Scielo databases. A search algorithm was used with the terms "bariatric surgery", "obesity", "exercise", "post-surgical", "exercise prescription" and "physical activity", in English and Spanish. Clinical trials and observational studies published between 2010-2020, where there was exercise prescription after bariatric surgery, were included. A total of13 studies were analyzed. The recommended parameters included frequency 3 to 5 times per week, aerobic intensity between 60-85% of HRmax or 60-70% of VO2max, anaerobic intensity between 60-85% of 1 repetition maximum (1RM), and duration period between 12 and 18 weeks. Exercise is an important tool to maintain and reduce weight, attenuate sedentary lifestyle and improve quality of life in post-bariatric surgery patients with safe and effective results as long as the suggested levels of intensity, frequency, duration, type and progression are guaranteed.

Introducción: durante el postoperatorio por cirugía de cadera, el delírium es la complicación más frecuente en los pacientes ancianos, con una alta incidencia y prevalencia, la cual lleva a una alta morbimortalidad en ellos. Objetivo: identificar los factores asociados con delírium en pacientes ancianos durante su posto-peratorio de cirugía por fractura de cadera. Materiales y métodos: se realizó un estudio observacional analítico de corte retrospectivo con pacientes hospitalizados por el grupo de Ortogeriatría en el Hospital Universitario San Ignacio en Bogotá (Colombia), entre enero de 2017 y septiembre de 2020. Resultados: se incluyeron 210 personas, en quienes se documentó una incidencia de delírium del 28.57 %. En el análisis bivariado, las variables con asociación estadísticamente significativa fueron edad, dependencia, demencia previa, malnutrición, polifarmacia y tipo de anestesia. Sin embargo, en el análisis multivariado, las dos variables con asociación estadísticamente significativa fueron edad (OR: 1.05; IC95 %: 1.01-1.10; p = 0.014) y demencia (OR: 2.83; IC95 %: 1.28-6.27; p = 0.010). Conclusión: las variables asociadas con delírium reportadas en el estudio concuerdan con la literatura ya publicada. Esto abre las puertas a futuras investigaciones no solo para identificar nuevos o más factores asociados, sino también para adoptar medidas en conjunto con los programas de ortogeriatría, a fin de intervenir estos factores y, de esta manera, poder disminuir la incidencia y prevalencia del delírium y, por ende, su morbimortalidad.


Introduction: Post-operative delirium after hip surgery is the most common complication among the elderly, with a high incidence and prevalence, which leads to high morbidity and mortality rates among them. Objective: To identify the main factors associated with post-operative delirium after hip surgery among older adults. Materials and methods: a retrospective analytical observational study was conducted using data from patients hospitalized by the Orthogeriatric group at the San Ignacio University Hospital in Bogotá, Colombia, between January 2017 and September 2020. Results: 210 people were included in the study, with a documented incidence of delirium of 28.57% was documented. In the bivariate analysis, the variables with a statistically significant association with delirium were age, dependency, previous dementia, malnutrition, polypharmacy, and type of anesthesia used in the procedure. Nonetheless, in a multivariate analysis, the two variables with a statistically significant association were age (OR: 1.05; 95%CI: 1.01-1.10; p = 0.014) and dementia (OR: 2.83; 95% CI: 1.28-6.27; p = 0.010). Conclusion: the variables associated with postoperative delirium reported in our study align with the existing literature. This opens doors to future research not only to identify new or more risk factors, but also to adopt measures, jointly with the Orthogeriatric programs, to intervene such factors so that the incidence and prevalence of delirium can be reduced, and therefore, reduce the morbidity and mortality among the elderly.


Introdução: o delirium no pós-operatório de cirurgia de quadril é a complicação mais frequente em pacientes idosos, com alta incidência e prevalência, o que leva a alta morbimortalidade nos mesmos e, assim, o objetivo deste estudo foi identificar os fatores associados ao delirium em pacientes idosos no pós-operatório de cirurgia de fratura de quadril. Materiais e métodos: foi realizado um estudo observacional analítico retrospectivo com pacientes internados pelo grupo de Ortogeriatria do Hospital Universitário San Ignacio, em Bogotá, Colômbia, entre janeiro de 2017 e setembro de 2020. Resultados: foram incluídas 210 pessoas, nas quais foi documentado incidência de delirium de 28,57%. Na análise bivariada, as variáveis com associação estatisticamente significativa foram idade, dependência, demência prévia, desnutrição, polifarmácia e tipo de anestesia. Porém, na análise multivariada, as duas variáveis com associação estatisticamente significativa foram idade (or: 1,05; Ic 95% 1,01-1,10; p: 0,014) e demência (or: 2,83; Ic 95% 1,28-6,27, p: 0,010). Conclusão: as variáveis associadas ao delirium relatadas em nosso estudo concordam com a literatura publicada anteriormente. Isso abre as portas para pesquisas futuras não só para identificar novos ou mais fatores associados, mas também para adotar medidas em conjunto com programas de ortogeriatria para poder intervir nesses fatores e, assim, reduzir a incidência e prevalência de delirium e, portanto, a morbimortalidade

ABSTRACT

El proceso de atención en enfermería (PAE) es un método sistemático y organizado que requiere de un entrenamiento específico, tener conocimientos y habilidades prácticas que proporcionan las herramientas para brindar cuidado abordando las dimensiones de forma holística a partir de una interacción directa con el paciente, la familia y el entorno social. Se presenta el PAE de una persona mayor, femenina, de 65 años de edad, con pluripatologías: síndrome purpúrico, monoparesia de miembro inferior derecho y síndrome convulsivo, reintervenida quirúrgicamente de un reemplazo de cadera derecha. Se plantea el PAE y sus cinco etapas: valoración, diagnóstico, planeación, ejecución y evaluación; siguiendo la valoración por dominios. El plan de cuidados se realiza con el enfoque de mapa de cuidados en la situación quirúrgica, diagnóstico NANDA International, lnc. La evaluación de intervenciones NIC (Nursing Interventions Classification) y resultados NOC (Nursing Outcomes Classification).


The Nursing Care Process (NCP) is a systematic and organized method that requires specific training, knowledge and practical skills that provide the tools needed to provide care by addressing the dimensions holistically from direct interaction with the patient, the family and social environment. Te NCP of a 65-year-old female elderly person with multiple pathologies; purpuric syndrome, right lower limb monoparesis and convulsive syndrome, who underwent surgery for a right hip replacement is presented. The Nursing Care Process (NCP) and its five stages are proposed: assessment, diagnosis, Outcomes/ Planning, Implementation and evaluation; following the valuation by domains. The care plan was carried out with the care map approach, NANDA Internacional Inc. Te evaluation of NIC (Nursing Interventions Classification) interventions and NOC (Nursing Outcomes Classification) results.

Abstract Objective The COVID-19 pandemic led to an unprecedented pause in elective surgeries, including shoulder arthroplasty. We sought to determine whether clinical and/or demographic differences would be seen between patients who presented for shoulder arthroplasty during the pandemic compared with the previous year (2019). Methods Institutional records were queried for patients who underwent shoulder replacement between March 1 and July 1 of 2019 and 2020. Demographics, range of motion, surgical duration, hospitalization time, discharge disposition, and postoperative management were analyzed. Results The mean duration of surgery was 160 ± 50 minutes in 2020 and 179 ± 54 minutes in 2019 (p= 0.13). The mean hospitalization time was 36 ± 13 hours in 2020 and 51 ± 40 hours in 2019 (p= 0.04). In 2019, 96% of the patients participated in physical therapy, while 71% did it in 2020 (p= 0.003). A total of 100% of the 2019 patients and 86% of the 2020 patients participated in an in-person postoperative follow-up (p= 0.006). The 2019 patients reported for an office visit on average 14 ± 11 days after surgery; the 2020 patients waited 25 ± 25 days to return for a follow-up (p= 0.10). Range of motion, age, American Society of Anesthesiologists (ASA) scores, and complication rates did not differ between the cohorts. Conclusion Patients presenting for surgery during the initial phase of the pandemic were demographically and clinically similar to 2019 patients. However, the length of stay was significantly reduced during the COVID-19 pandemic. Postoperative follow-up and physical therapy were delayed in 2020, but this did not lead to differences in complication or readmission rates compared with those of the 2019 cohort. Resumo Objetivo A pandemia de COVID-19 causou uma pausa sem precedentes em cirurgias eletivas, inclusive artroplastia de ombro. Procuramos determinar as possíveis diferenças clínicas e/ou demográficas entre os pacientes que realizaram artroplastia de ombro durante a pandemia em comparação com o ano anterior (2019). Métodos Os registros institucionais foram consultados para obtenção de informações sobre pacientes submetidos a artroplastia de ombro entre 1° de março a 1° de julho de 2019 e 2020. Dados demográficos, amplitude de movimento, duração da cirurgia, tempo de hospitalização, condições à alta e manejo pós-operatório foram analisados. Resultados O tempo médio de cirurgia foi de 160 ± 50 minutos em 2020 e de 179 ± 54 minutos em 2019 (p= 0,13). O tempo médio de internação foi de 36 ± 13 horas em 2020 e de 51 ± 40 horas em 2019 (p= 0,04). Em 2019, 96% dos pacientes fizeram fisioterapia, enquanto 71% o fizeram em 2020 (p= 0,003). Todos os pacientes de 2019 e 86% dos pacientes de 2020 participaram do acompanhamento pós-operatório presencial (p= 0,006). Os pacientes de 2019 retornaram para a consulta médica em média 14 ± 11 dias após a cirurgia; os pacientes de 2020 retornaram para o acompanhamento em 25 ± 25 dias (p= 0,10). A amplitude de movimento, a idade, a pontuação da American Society of Anesthesiologists (ASA, na sigla em inglês) e as taxas de complicações não diferiram entre as coortes. Conclusão Os pacientes submetidos a cirurgia na fase inicial da pandemia eram demográfica e clinicamente semelhantes aos pacientes de 2019. No entanto, o tempo de internação diminuiu de forma significativa durante a pandemia de COVID-19. O acompanhamento pós-operatório e a fisioterapia foram adiados em 2020, mas isso não levou a diferenças nas taxas de complicações ou de reinternações em comparação às da coorte de 2019. ABSTRACT Objective: To explain the rationale and protocol of the methods and analyses to be used in the LIVER-PAM randomized clinical trial, which seeks to understand whether a higher mean arterial pressure is capable of reducing the incidence of renal dysfunction postoperatively after liver transplantation. Methods: LIVER-PAM is an open-label, randomized, controlled, singlecenter clinical trial. Patients randomized to the intervention group will have a mean arterial pressure of 85 - 90mmHg in the initial 24 hours of postoperative management, while patients in the control group will have a mean arterial pressure of 65 - 70mmHg in the same period. A sample of 174 patients will be required to demonstrate a 20% reduction in the absolute incidence of renal dysfunction, with a power of 80% and an alpha of 0.05. Conclusion: If a 20% reduction in the absolute incidence of renal dysfunction in the postoperative period of liver transplantation is achieved with higher target mean arterial pressure in the first 24 hours, this would represent an inexpensive and simple therapy for improving current outcomes in the management of liver transplant patients. RESUMO Objetivo: Explicitar o racional e o protocolo de métodos e análises a serem utilizadas no ensaio clínico randomizado LIVER-PAM, que busca entender se um nível mais alto de pressão arterial média é capaz de reduzir a incidência de disfunção renal no pós-operatório de transplante hepático. Métodos: O LIVER-PAM é um estudo clínico randomizado, controlado, unicêntrico e aberto. Pacientes randomizados para o grupo intervenção terão como alvo de pressão arterial média 85 - 90mmHg nas 24 horas iniciais do manejo pós-operatório, enquanto pacientes do grupo controle terão como alvo de pressão arterial média 65 - 70mmHg no mesmo período. Uma amostra de 174 pacientes será necessária para demonstrar redução de 20% na incidência absoluta de disfunção renal, com poder de 80% e alfa de 0,05. Conclusão: Se a redução de 20% da incidência absoluta de disfunção renal no pós-operatório de transplante hepático for obtida com alvos maiores de pressão arterial média nas primeiras 24 horas, o manejo do paciente nesse cenário encontraria uma terapia barata e simples para a melhoria dos desfechos atuais. ABSTRACT

Introducción: La terapia de reemplazo renal continua es un procedimiento novedoso que se aplica al tratamiento de la falla renal aguda en el paciente grave, así como del síndrome de respuesta inflamatoria sistémica. Se aplica de preferencia en aquellos pacientes que cursan con deterioro hemodinámico en los cuales el tratamiento dialítico intermitente puede ser perjudicial por la relativa rapidez con que se produce el recambio. No obstante, no existe consenso en cuanto a su uso precoz. Objetivo: Presentar el caso de un paciente grave en la que se realizó terapia de reemplazo renal continua precoz. Presentación de caso: Paciente de 48 años de edad, con antecedentes de enfermedad renal crónica grado V, en régimen de hemodiálisis intermitente, hipertensión arterial y obesidad, que ingresó a la unidad de cuidados intensivos cardiovasculares del Hospital "Hermanos Ameijeiras", en el posoperatorio inmediato y complicado de cirugía de sustitución valvular aórtica; en la cual la terapia de reemplazo renal continua precoz, resultó ser eficiente para el tratamiento de la falla renal crónica agudizada, ya que permitió mantener un buen control de la hemodinámica del medio interno y la volemia. Conclusiones: La experiencia mostrada aporta elementos a favor de la seguridad del tratamiento, que lo hace recomendable en el paciente grave poscirugía cardiovascular, como parte del arsenal terapéutico a utilizar(AU)


Introduction: continuous renal replacement therapy is a novel procedure that is applied to the treatment of acute renal failure in critically ill patients, as well as systemic inflammatory response syndrome. It is preferably applied in those patients with hemodynamic deterioration in whom intermittent dialysis treatment can be harmful due to the relative speed with which replacement occurs. However, there is no consensus regarding its early use. Objective: to present the case of a seriously ill patient who underwent early continuous renal replacement therapy. Case report: a 48-year-old patient, with a history of grade V chronic kidney disease, on an intermittent hemodialysis regimen, high blood pressure and obesity, who was admitted to the Cardiovascular Intensive Care Unit of the Hermanos Ameijeiras Hospital in the immediate postoperative period and complicated by aortic valve replacement surgery; in which early continuous renal replacement therapy turned out to be efficient for the treatment of acute chronic renal failure, since it allowed maintaining good control of hemodynamics, the internal environment and blood volume. Conclusions: the experience shown provides elements in favor of the safety of the treatment, which makes it recommendable in the seriously ill patient after cardiovascular surgery, as part of the therapeutic arsenal to be used(AU)

Resumo Objetivo Analisar o conteúdo e usabilidade de um protótipo de aplicativo móvel para apoiar a educação do paciente no pré-operatório de revascularização miocárdica. Métodos Estudo metodológico, quantitativo, baseado no referencial design instrucional contextualizado. Na etapa de análise realizou-se revisão de escopo e um estudo qualitativo com 13 pacientes com a finalidade de identificar o conteúdo para compor o aplicativo. Na etapa de design e desenvolvimento estruturou-se o conteúdo. Durante a implementação, procedeu-se a configuração dos recursos e, na avaliação, utilizou-se a técnica Delphi, com a avaliação do conteúdo a partir dos critérios de Pasquali e a usabilidade do aplicativo por meio do ERGOLIST por 20 juízes na primeira rodada e 16 na segunda. Usou-se, para análise, o coeficiente de validade de conteúdo, considerado válido acima de 0,8 e o percentual de concordância igual ou superior a 80%. Realizou-se o teste binomial em cada item para determinar o nível de significância (p<0,05). Resultados A análise do conteúdo resultou em um coeficiente de validade de conteúdo superior a 0,8 e percentual de concordância superior a 80% em todos os critérios analisados. Já a usabilidade, apresentou percentual superior a 90% em todos os itens. Todos os itens analisados apresentaram significância estatística. O aplicativo OrientaRVM foi composto por 90 telas que formam o menu inicial e nove seções: entenda mais sobre o coração; entenda a sua doença; entenda a cirurgia de ponte de safena ou ponte mamária; cuidados antes da cirurgia; cuidados após a cirurgia; reabilitação e mudanças no estilo de vida; registros do paciente; agenda; quiz. Conclusão O OrientaRVM apresenta conteúdo confiável, funcionalidade adequada e foi recomendado para ser utilizado como recurso auxiliar na educação do paciente antes da revascularização miocárdica.

Por otro lado, la usabilidad presentó un porcentaje superior al 90 % en todos los ítems. Todos los ítems analizados presentaron significación estadística. La aplicación OrientaRVM estuvo compuesta por 90 pantallas que forman el menú inicial y nueve secciones: entienda más sobre el corazón; entienda su enfermedad; entienda la cirugía de bypass coronario o puente mamario; cuidados antes de la cirugía; cuidados después de la cirugía; rehabilitación y cambios en el estilo de vida; registros del paciente; agenda; quiz. Conclusión OrientaRVM presenta contenido confiable, funcionalidad adecuada y fue recomendado para su uso como recurso auxiliar en la educación del paciente antes de la revascularización miocárdica.


Abstract Objective To analyze the content and usability of a mobile application prototype to support patient education in the preoperative period of myocardial revascularization. Methods This is a quantitative methodological study based on the contextualized instructional design framework. In the analysis stage, a scoping review and a qualitative study were carried out with 13 patients to identify the content to compose the application. In the design and development stage, content was structured. In the implementation stage, resources were configured. In the assessment stage, the Delphi technique was used, with content assessment from the Pasquali criteria and application usability through the ERGOLIST by 20 judges in the first round and 16 in the second. For analysis, the Content Validity Coefficient was used, considered valid above 0.8 and the percentage of agreement equal to or greater than 80%. The binomial test was performed on each item to determine the significance level (p<0.05). Results Content analysis resulted in a Content Validity Coefficient greater than 0.8 and a percentage of agreement greater than 80% in all analyzed criteria. Usability, on the other hand, presented a percentage greater than 90% in all items. All items analyzed were statistically significant. The OrientaRVM application was composed of 90 screens that form the initial menu and nine sections: understand more about the heart; understand your illness; understand coronary artery bypass graft surgery or breast bypass surgery; care before surgery; care after surgery; rehabilitation and lifestyle changes; patient records; schedule; quiz. Conclusion OrientaRVM presents reliable content, adequate functionality and was recommended to be used as an auxiliary resource in patient education before myocardial revascularization.

ABSTRACT

Introducción: El tromboembolismo pulmonar es una enfermedad de relevancia clínica, en pacientes que se le han realizado intervenciones quirúrgicas. La tasa de mortalidad por esta causa ha disminuido en los últimos años. Objetivo: Describir el manejo diagnóstico y terapéutico del tromboembolismo pulmonar de riesgo intermedio. Caso clínico: Paciente masculino de 62 años de edad que se encontraba ingresado por haber sido operado de una apendicitis gangrenosa. En el posoperatorio mediato comenzó con cuadro de dificultad respiratoria, se realizaron exámenes complementarios incluido ecocardiograma transtorácico, donde se evidenció el signo de McConnell, por lo que se diagnosticó un tromboembolismo pulmonar de riesgo intermedio. El paciente se somete al tratamiento trombolítico estandarizado, luego de lo cual la clínica y los parámetros ecocardiográficos que evolucionaron hacia la mejoría. Conclusiones: Se describió el diagnóstico y tratamiento del tromboembolismo pulmonar de riesgo intermedio en el posoperatorio mediato de apendicectomía, donde se evidenció la utilidad del uso del ecocardiograma doppler transtorácico con la identificación de los signos característicos y la aplicación exitosa de trombolisis sistémica.


Introduction: Pulmonary thromboembolism (PTE) is an entity of clinical relevance in patients in settings related to surgical interventions. The mortality rate from this cause has decreased in recent years. Objective: To describe the diagnostic and therapeutic management of intermediate risk pulmonary thromboembolism. Clinical case: A 62-year-old male patient who was hospitalized for having been operated on for gangrenous appendicitis. In the immediate postoperative period, he begins with symptoms of respiratory distress, complementary tests are performed, including a transthoracic echocardiogram, where McConnell's sign is evident, for which an intermediate-risk pulmonary thromboembolism is diagnosed. The patient undergoes standardized thrombolytic treatment, after which the clinical and echocardiographic parameters evolve towards improvement. Conclusions: The diagnosis and treatment of intermediate risk pulmonary thromboembolism in the immediate postoperative period of appendectomy was described. The usefulness of the use of transthoracic Doppler echocardiography was evidenced with the identification of the characteristic signs and the successful application of systemic thrombolysis.

Objetivo: avaliar os fatores clínicos preditores para o ressecamento ocular e para o diagnóstico de enfermagem Risco de ressecamento ocular em pacientes no pós-operatório em Unidade de Recuperação Pós-Anestésica. Método: estudo transversal realizado entre maio e agosto de 2017. Amostra foi de 157 pacientes adultos submetidos a procedimentos cirúrgicos eletivos. Os dados foram analisados mediante estatística descritiva e inferencial. Resultados: o diagnóstico clínico de ressecamento ocular prevaleceu em 85,35% dos pacientes, e o diagnóstico de enfermagem Risco de ressecamento ocular em 14,65%. Observou-se diferenças estatísticas entre a hiperemia, lacrimejamento excessivo e teste de Schirmer insuficiente com o diagnóstico de ressecamento ocular no olho direito e com o diagnóstico Risco de ressecamento ocular. No olho esquerdo o teste de Schirmer foi insuficiente. Conclusão: a hiperemia, o lacrimejamento excessivo e o teste de Schirmer insuficiente são fatores clínicos preditores relevantes no perioperatório para promover medidas preventivas e/ou detectar precocemente o ressecamento ocular.


Objetivo: evaluar los predictores clínicos de ojo seco y el diagnóstico de enfermería Riesgo de ojo seco en pacientes postoperados en la Unidad de Recuperación Postanestésica. Material y método: estudio transversal realizado entre mayo y agosto de 2017. La muestra fue de 157 pacientes adultos sometidos a procedimientos quirúrgicos electivos. Los datos se analizaron mediante estadística descriptiva e inferencial. Resultados: el diagnóstico clínico de ojo seco predominó en el 85,35% de los pacientes, y el diagnóstico de enfermería Riesgo de ojo seco en el 14,65%. Se observaron diferencias estadísticas entre la hiperemia, el lagrimeo excesivo y el test de Schirmer insuficiente con el diagnóstico de sequedad ocular en el ojo derecho y con el diagnóstico Riesgo de ojo seco. En el ojo izquierdo el test de Schirmer fue insuficiente. Conclusión: la hiperemia, el lagrimeo excesivo y el test de Schirmer insuficiente son predictores clínicos perioperatorios relevantes para promover medidas preventivas y/o la detección precoz del ojo seco.


Objective: to evaluate the clinical predictors for dry eye and the nursing diagnosis Risk of dry eye in postoperative patients in the Post-Anesthesia Care Unit. Method: cross-sectional study conducted between May and August, 2017. The sample was 157 adult patients undergoing elective surgical procedures. Data were analyzed using descriptive and inferential statistics. Results: the clinical diagnosis of dry eye prevailed in 85.35% of patients, and the nursing diagnosis Risk of dry eye in 14.65%. Resumo Objetivo Avaliar o conhecimento de pacientes e profissionais de saúde sobre os protocolos de vacinação no pós-transplante hepático antes e após a aplicação de uma estratégia educativa de conscientização nessa população. Métodos Os pacientes (n=124) foram submetidos à intervenção educativa através do acesso a uma página web com vídeos educativos e, para os profissionais de saúde (n=111), através de um simpósio e acesso à informação na página virtual do projeto. Para analisar o efeito da intervenção, análises qualitativas de conhecimento foram realizadas por meio de questionários antes e depois das intervenções. Resultados Entre os pacientes, predominou o sexo masculino (66,9%) e a média de idade foi de 55,2 anos (DP + 15,9). 82,2% dos pacientes procuraram uma UBS para serem vacinados e 13,7% deles, os CRIEs (Centro de Referência para Imunobiológicos Especiais). Apenas 46,7% receberam orientações sobre vacinas após o transplante hepático. Dos 111 questionários respondidos pelos profissionais de saúde, 46,5% não consultaram a carteira de vacinas, 61,3% encaminharam os pacientes para UBS e 38,7%, para um CRIE. Na análise pós-intervenção, 66,1% dos pacientes assistiram a vídeos educativos sobre vacinação. Destes, 62,2% disseram ter melhorado seu entendimento sobre as vacinas e 91,4% se sentem mais seguros para vacinar. Após a intervenção educativa, 45 profissionais de saúde responderam ao questionário. 30,4% afirmaram saber quais vacinas prescrever e 67,4% indicaram vacinas para familiares de pacientes. Conclusão A estratégia educacional proposta aplicada neste estudo mostrou aumentar a conscientização sobre os protocolos de imunização pós-transplante hepático. Isso pode contribuir para evitar o risco potencial de falta de informação e não abordagem da vacinação pelos profissionais de saúde.

Métodos Los pacientes (n=124) fueron sometidos a la intervención educativa mediante el acceso a un sitio web con videos educativos, y los profesionales de la salud (n=11), mediante un simposio y el acceso a información en la página web del proyecto. Para analizar el efecto de la intervención, se realizaron análisis cualitativos de conocimiento con cuestionaros antes y después de la intervención. Resultados Entre los pacientes, el sexo masculino fue predominante (66,9 %) y la edad promedio fue 52,2 años (SD + 15,9). El 82,2 % de los pacientes fue a una Unidad Básica de Salud (UBS) para recibir la vacuna y el 13,7 % de ellos acudió a un Centro de Referencia para Inmunobiológicos Especiales (CRIE). Solo el 46,7 % recibió instrucciones sobre vacunación tras el trasplante hepático. De los 111 cuestionarios respondidos por profesionales de la salud, el 46,5 % no consultó el catálogo de vacunas, el 61,3 % derivó a los pacientes a una UBS y el 38,7 % a un CRIE. En el análisis posintervención, el 66,1 % de los pacientes miró los videos educativos sobre vacunación. De ellos, el 62,2 % mencionó haber mejorado su comprensión sobre vacunas y el 91,4 % se sintió más seguro para vacunarse. Después de la intervención educativa, 45 profesionales de la salud respondieron el cuestionario. El 30,4 % afirmó saber qué vacunas prescribir y el 67,4 % recomendó vacunas a familiares de los pacientes. Conclusión La estrategia educativa propuesta aplicada en este estudio demostró un aumento de conocimiento sobre los protocolos de inmunización tras un trasplante hepático. Esto pude ayudar a evitar el riesgo potencial de la falta de información y el no abordar el tema de la vacunación por parte de los profesionales de la salud.


Abstract Objective To evaluate patients' and healthcare professionals' knowledge about vaccination protocols in post-liver transplantation before and after applying an educational awareness strategy in this population. Methods Patients (n=124) underwent the educational intervention through access to a webpage containing educational videos and, for health professionals (n=111), through a symposium and access to information on the project's virtual page. To analyze the effect of the intervention, qualitative analyses of knowledge were carried out using questionnaires before and after the interventions. Results Among patients, males were predominant (66.9%) and the mean age was 55.2 years old (SD + 15.9). 82.2% of patients visited a UBS to be vaccinated and 13.7% of them the CRIEs (Reference Center for Special Immunobiologicals). Only 46.7% received orientation about vaccines after liver transplantation. From the 111 questionnaires answered by health professionals, 46.5% did not check the vaccine portfolio, 61.3% referred patients to UBS and 38.7% to a CRIE. In the post-intervention analysis, 66.1% of patients watched educational videos about vaccination. Of these, 62.2% said they had improved their understanding about vaccines and 91.4% feel safer to vaccinate. After the educational intervention, 45 health professionals answered the questionnaire. 30.4% said they knew which vaccines to prescribe, and 67.4% recommended vaccines to patients' relatives. Conclusion The proposed educational strategy applied in this study shown to increase awareness regarding the post-liver transplant immunization protocols. This may contribute to avoiding the potential risk of lack of information and failure to address vaccination by healthcare professionals.

ABSTRACT

Objetivo: avaliar a intensidade e o desconforto provocados pela sede em pacientes em pós-operatório imediato. Método: estudo exploratório-descritivo, desenvolvido com pacientes internados na sala de recuperação pós-anestésica de um hospital público paranaense. Avaliaram-se indivíduos maiores de 18 anos, de ambos os sexos, com cognitivo preservado, internados de junho de 2021 a janeiro de 2022. A intensidade e o desconforto da sede foram mensurados por escalas específicas. Resultados: avaliaram-se 150 pacientes, com média de 43,9 anos. A maioria era do sexo masculino (65,3%), sem comorbidades (68,7%), submetida à raquianestesia (58%) e cirurgia ortopédica (59,3%), com soroterapia em curso (92,7%). O tempo médio de cirurgia foi 1,5 hora e 14,6 horas de jejum; 72,7% da amostra verbalizou sede, sendo que 37,6% queixaram-se de forma espontânea. Conclusão: os participantes de pesquisa apresentaram intensidade (6,6) e desconforto (7,6) moderados de sede no pós-operatório, tornando-se necessário discutir protocolos institucionais de intervenção para diminuir tal evento.


Objective: to evaluate the intensity and discomfort caused by thirst in patients in the immediate postoperative period. Method: exploratory-descriptive study developed with patients hospitalized in the post-anesthetic recovery room of a public hospital in Paraná. Individuals over 18 years of age, of both sexes, with preserved cognitive function, hospitalized from June 2021 to January 2022, were evaluated. Thirst intensity and discomfort were measured by specific scales. Results: 150 patients were evaluated, with a mean of 43.9 years. Most were male (65.3%), without comorbidities (68.7%), underwent spinal anesthesia (58%) and orthopedic surgery (59.3%), with ongoing serotherapy (92.7%). The mean surgery time was 1.5 hours and 14.6 hours of fasting; 72.7% of the sample verbalized thirst, and 37.6% complained spontaneously. Conclusion: the research participants presented moderate intensity (6.6) and discomfort (7.6) of thirst in the postoperative period, making it necessary to discuss institutional intervention protocols to reduce such event.

Se midió la intensidad y el malestar de la sed mediante escalas específicas. Resultados: Se evaluaron 150 pacientes, con una edad media de 43,9 años. La mayoría eran hombres (65,3%), sin comorbilidades (68,7%), con anestesia espinal (58%) y cirugía ortopédica (59,3%), con sueroterapia en curso (92,7%). El tiempo promedio de cirugía fue de 1,5 horas y 14,6 horas de ayuno; El 72,7% de la muestra verbalizó sed, con un 37,6% quejándose espontáneamente. Conclusión: los participantes de la investigación presentaron moderada intensidad (6,6) y malestar (7,6) de la sed en el postoperatorio, siendo necesario discutir protocolos de intervención institucional para la reducción de ese evento.

ABSTRACT Introduction: We compared transatrial closure, tricuspid valve septal detachment, and tricuspid valve chordal detachment techniques for ventricular septal defect (VSD) closure. Methods: Patients who had VSD closure with three different techniques in our clinic between September 2016 and December 2020 were retrospectively reviewed. A total of 117 patients were included in the study. The patients were divided into three groups: group 1, classical transatrial closure; group 2, closure with tricuspid valve septal detachment; and group 3, closure with tricuspid valve chordal detachment. The groups were evaluated by serial transthoracic echocardiography (preoperative, postoperative 1st day, postoperative 1st month). Cardiac rhythm checks and recordings were performed. Results: No residual VSD was observed in early or late periods in any of the groups whose VSD closure was performed with the three different techniques. No severe tricuspid regurgitation (TR) was detected during the early and late postoperative periods of all operating procedures. When the groups were compared in terms of early/late TR after the operation (without TR+trace amount of TR and mild TR+moderate TR were compared), no statistically significant difference was found (P>0,05; P=0,969 and P>0,05; P=0,502). Conclusion: In this study, we found no statistically significant difference between three VSD closure techniques in terms of early TR, late TR, residual VSD, and permanent atrioventricular complete block during postoperative period. We hope that our results will be supported by the results of researches that are being made about this subject in large series.

ABSTRACT

Objective:To investigate the effects of the timing of chemotherapy after breast cancer surgery on patient's immune function and quality of life.Methods:A total of 100 patients who underwent modified radical mastectomy for breast cancer from January 2017 to January 2019 in Jining No. 1 People's Hospital were included in this study. These patients were randomly divided into a control group and an early chemotherapy group ( n = 50/group). Patients in the control group underwent chemotherapy 4-6 weeks after surgery. Patients in the early chemotherapy group received chemotherapy 2 weeks after surgery. The chemotherapy regimens were the same in the two groups. The levels of CD 4+, CD 8+, CD 4+/CD 8+, immunoglobulin A (IgA), and immunoglobulin G (IgG) were measured before and after chemotherapy in each group. Chemotherapy-related reverse reactions and infections were recorded. The quality of life was evaluated in each group at the last follow-up. Results:Before chemotherapy, there were no significant differences in CD 4+, CD 8+, CD 4+/CD 8+, IgA, and IgG levels between the two groups (all P > 0.05). After chemotherapy, CD 4+ and CD 4+/CD 8+ levels in the early chemotherapy group were (51.76 ± 5.21)% and (2.00 ± 0.25), respectively, which were significantly higher than (48.21 ± 4.78)% and (1.70 ± 0.21) in the control group ( t = 3.55, 4.98, both P < 0.05). After chemotherapy, the CD 8+ level in the early chemotherapy group was (25.93±2.43)%, which was significantly lower than (28.29 ± 2.31)% in the control group ( t = 6.50, P < 0.05). Serum IgA and IgG levels in the early chemotherapy group were (3.24 ± 0.38) g/L and (9.27 ± 1.04) g/L, respectively, which were significantly higher than (2.75 ± 0.37) g/L and (8.43 ± 0.97) g/L in the control group ( t = 6.53, 4.18, both P < 0.05). During chemotherapy, there was no significant difference in the incidence of reverse reactions between the two groups (all P > 0.05). The incidence of infections was significantly lower in the early chemotherapy group than the control group ( P < 0.05). At the last follow-up, generic quality of life inventory-74 scores in the early chemotherapy group were significantly higher than those in the control group (all P < 0.05). Conclusion:Early chemotherapy can markedly reduce the effects of chemotherapy on the immune function of patients after breast cancer surgery, decrease the incidence of infections, and improve quality of life.

Introdução: Pacientes submetidos a grandes cirurgias abdominais apresentam riscos de complicações pós-operatórias. A mobilização precoce vem sendo implementada e cada vez mais aplicada, no intuito de prevenir esses eventos. Objetivo: Demonstrar se a mobilização precoce está associada à melhor funcionalidade no pós-operatório de cirurgias abdominais. Métodos: Revisão integrativa de literatura realizada por meio de uma busca bibliográfica junto aos bancos de dados: BVS, Scielo, PedRO e Pubmed por meio dos descritores: mobilização precoce, deambulação precoce, cuidados pós-operatórios, período pós-operatório, estado funcional, exercício físico, reabilitação, funcionalidade e cirurgia abdominal, nos idiomas inglês, português e espanhol. Resultados: A amostra final foi constituída por 08 artigos científicos, que foram estruturados em forma de quadro para apresentação de suas principais características, dos métodos e os principais resultados. Conclusão: A mobilização precoce está associada ao retorno rápido à funcionalidade da linha de base pré- operatória, as atividades de vida diária, independência funcional, além do tempo de internação mais curto e menor duração dos desagradáveis sintomas pós-operatórios.


Introduction: Patients undergoing major abdominal surgery are at risk of postoperative complications. Early mobilization has been implemented and increasingly applied in order to prevent these events. Objective: to demonstrate whether early mobilization is associated with better functionality in the postoperative period of abdominal surgeries. Methods: an integrative literature review carried out through a literature search in the following databases: BVS, Scielo, PedRO and Pubmed using the descriptors: early mobilization, early ambulation, postoperative care, postoperative period, functional status, physical exercise, rehabilitation, functionality and abdominal surgery, in English, Portuguese and Spanish. Results: The final sample consisted of 08 scientific articles, which were structured in the form of a table to present their main characteristics, methods and main results. Conclusion: Early mobilization interferes with the rapid return to preoperative baseline functionality, activities of daily living, functional independence, in addition to a shorter hospital stay and shorter duration of unpleasant postoperative symptoms.

Background : Breast cancer is the most common type of cancer amongst women. Amongst the many factors which affect the outcome of breast cancer surgeries is Diabetes Mellitus. Diabetes Mellitus is an ongoing problem Worldwide. It is a predictor for Postoperative complications in women who undergo surgical management for breast cancer. Aims And Objectives : (1) To compare the rate of postoperative complications in Diabetic and non-diabetic patients undergoing breast cancer surgeries. (2) To compare the in hospital outcomes in diabetic and non diabetic patients. (3) To study the pre and Postoperative control of sugars in Diabetic patients undergoing breast cancer surgeries. Methods : Data will be collected from the in patient files and the surgery case completion register in the Medical records department of hospitals attached to Bangalore Medical College and the data will be retrospectively scrutinized for a period of 3 years from 01/09/2018 to 30/08/2021. Patients diagnosed with breast cancer and planned for surgical management of the same were included in the study. Results : A total of 100 breast cancer patients were studied. 34 were Diabetic of which 16 had uncontrolled sugars pre operatively. Out of which only 5 patients remained to have uncontrolled sugars Postoperatively. A total of 27 patients developed Postoperative complications. 13 were Diabetic of which 9 had uncontrolled sugars. Of the diabetic patients a total 27 had a drain duration of more than 7 days. Conclusion : Diabetic women who undergo breast cancer surgeries at an increased risk of complications post operatively, more so if they have uncontrolled sugars. In hospital outcomes too are different in patients with Diabetes as in the duration of drain thereby the total in patient days were higher in patients with diabetes. Also patients with uncontrolled sugars saw an improvement in their sugars postoperatively.

Introducción. Los colgajos del territorio de la arteria submentoniana pueden ser utilizados como un colgajo cutáneo, musculofacial y osteocutáneo, realizando cierres primarios del defecto del sitio donante, sin generar defectos funcionales ni estéticos mayores. Métodos. Describir la experiencia de nuestro equipo quirúrgico, las complicaciones relacionadas con el uso del colgajo y los resultados oncológicos, así como los desenlaces tardíos durante el seguimiento de los pacientes incluidos en el estudio. Resultados. Se incluyeron veintiún pacientes, con una edad media de 66 años (rango 52 - 86), con patología oncológica de lengua, labio inferior, paladar blando, nariz, órbita y orofaringe. Todos los pacientes fueron sometidos a disección selectiva ipsilateral del cuello, tras la extracción del colgajo y en todos los casos se preservó el nervio mandibular marginal. Se registraron complicaciones como la necrosis parcial. La estancia hospitalaria media fue de 8 días.Conclusiones. El colgajo de la arteria submentoniana ha mostrado resultados favorables debido a su uso versátil, amplio arco de rotación, color y baja morbilidad del sitio donante. Se recomienda realizar estudios más robustos, que incluyan la experiencia de diversos especialistas en países que compartan las mismas limitaciones técnicas y características sociodemográficas.


Introduction. Flaps from the territory of the submental artery can be used as a cutaneous, musculofacial and osteocutaneous flap, performing primary closure of the donor site defect, without generating major functional or aesthetic defects. Methods. To describe the experience of the same surgical team, the complications related to the use of the flap and the oncological results, as well as the late outcomes during the follow-up of the patients included in the study. Results. Twenty-one patients with a mean age of 66 years (range: 52-86), with oncological pathology of the tongue, lower lip, soft palate, nose, orbit, and oropharynx were included. All patients underwent ipsilateral selective neck dissection after flap removal, and in all cases the marginal mandibular nerve was preserved. Complications such as partial necrosis were recorded. The mean hospital stay was 8 days. Conclusions. The submental artery flap has shown favorable results due to its versatile use, wide arc of rotation, color, and low donor site morbidity. More robust studies are recommended, including the experience of various specialists in countries sharing the same technical limitations and sociodemographic characteristics.

Abstract Introduction: In developing countries like India, it is common for late presentation of Tetralogy of Fallot (TOF) patients to a hospital as compared to that of developed countries. The objective of this study is to analyze the surgical outcome of TOF patients with age > 15 years. Methods: This is a retrospective descriptive study of the surgical outcomes of 45 adult patients undergoing correction for TOF. Epidemiology, symptomology, and preoperative evaluation were performed. Results: Most of the patients were male (33 [73%]). The median age was 21 years. A total of 42 (93.33%) patients had subaortic ventricular septal defect (VSD), while three (6.6%) patients presented with doubly committed VSD. The most common type of right ventricular outflow tract (RVOT) obstruction was combined infundibular and valvular types, accounting for 34 cases (75.5%). Six patients had infundibular RVOT obstruction, while three patients (6.6%) had predominantly valvular pulmonary stenosis. We performed trans-right atrial repair in 33 patients. Right atrium-pulmonary artery approach was used in five patients (11.1%). The most common postoperative complication was right bundle branch block, seen in 14 patients, with a mortality rate of 2% in the early postoperative period. We achieved excellent early and midterm survival results and significant improvement in functions and disease-free quality of life. Conclusion: Intracardiac repair in adult TOF can be performed with low mortality, less residual RVOT obstruction, and need for revision of RVOT far less frequent by using the Jhajhria Infundibular Resection Adequacy Assessment technique (JIRAAT) to assess for adequacy of infundibular resection.
